This Consumer Health Data Policy describes how RivenMD+ collects, uses, and shares "consumer health data" of residents of Nevada and Washington, as that term is defined under the privacy laws of those states. It applies to consumer health data collected by or for RivenMD+ through our websites, including www.rivenmd.com, our email messages, and our in-person or offline interactions that involve consumer health data (the "Website").

This Consumer Health Data Policy supplements our Privacy Policy and applies solely to the consumer health data of Nevada and Washington residents. By providing us with consumer health data or using the Website, you agree to the collection, use, and disclosure of your consumer health data as described here. If you do not agree, do not provide us with consumer health data and do not use the Website.

RivenMD+ ("RivenMD+", "we", "our", or "us") operates from a principal place of business at 22818 Paradise Gate Dr, Spring, TX 77373, together with its affiliates and subsidiaries.

1. Consumer health data we may collect

Depending on how you interact with us and the choices you make, we (and the vendors that perform services on our behalf) may collect the following categories of consumer health data:

  • Health-related information that, with your authorization, we receive from your telehealth provider in connection with the Telehealth Platform.

2. Sources of consumer health data

We may collect consumer health data directly from you, for example when you register for a newsletter, join a waitlist, or otherwise submit it to us. We and our vendors may also collect it passively through cookies and similar technologies as you interact with our Website and emails, as described in our Cookie Policy. With your authorization, we may receive consumer health data from your health care provider in connection with the Telehealth Platform, as described in our Privacy Policy and Terms of Service.

3. How we use consumer health data

We may collect and use consumer health data to:
